Testing Consent Form: Give consent to provide urine, blood and or breath sample for drug and/or alcohol testing.

Clinic Information Sheet: A consent for treatment form.

Clinic Physical Information Sheet: Gives consent for a physical and releases the results to employer

Patient Hearing History Questionnaire: Assesses hearing and authorizes disclosure of the Audiogram Test results to employer.
